For those who need to obtain Japanese visa, please fill in and send the form below to spinmechanics2@imr.tohoku.ac.jp with a copy of the photo page of your passport. We will send you the visa document before the middle of May, 2014. If you have any requests, especially about the timing of receiving the document for your visa, please write in the "Note" section of the form.
